This form is intended to be completed in cases where oversized baggage is inadequately packed and handed over at check-in without a protective cover, box, or packaging that provides adequate protection.

I, the undersigned, the above named passenger (parent/guardian) of the named passenger (hereinafter referred to as “Passenger” hereby agree to release and discharge Mavi Gök Havacılık A.Ş (MGA) (hereinafter referred as “MGA”) including its agents, officers, and employees from any and all liabilities which may be consequential as the result of the travel and/or carriage by MGA and agree to indemnity and hold MGA harmless from any and all damages and losses sustained by MGA and any all actions, claims and proceedings against MGA by any third party or parties including all costs, losses, damages, charges and expenses incidental thereto which may be suffered by MGA as the result of travel and/or carriage by MGA specified above due to conditions specified below and/or any possible consequence thereof.

I, understand that MGA will not provide any special assistance to the above mentioned passenger(s).I hereby indemnity and hold harmless MGA and/or other related carriers, their personnel and agents and against any loss or damage sustained and/or any costs and expenses incurred by them in connection with the above mentioned passenger(s) travel or resulting therefrom and to release them from any and all liability other than as set forth in MGA General Conditions of Carriage and applicable Tariffs on www.mga.aero
